FINAL WISHES 'The Family Guide to End of Life Decisions'
Having discussions with loved ones about medical decisions or health concerns that impact end-of life decisions can be difficult. Complicating the situation is that these decisions usually become necessary at an unexpected time for the patient and their family. In my practice, have talked to many family members that shared how they wished the discussion about these choices had occurred before they had to make the decisions. My workbook, Final Wishes, will help you determine what is important to you and offer tips on how to discuss your wishes with your loved ones or health care providers.
I help families understand advance health care planning, treatments and decisions about treatments for cure or comfort. My purpose is to encourage these discussions among interested parties before a crisis occurs and have the person directly impacted by the serious health issue participate in the decision-making process. This will provide an improved outcome and experience as one approaches the end of life and ultimately improve the overall quality of life.
